Referrals
Referrals can be made by anyone and involve a phone call. Carers can refer themselves if necessary. Staff will then make arrangements for a home visit or to meet at a convenient place to assess what support is needed.
From that point we will tailor the support a carer needs, providing services available from us or other local organisations relevant to condition or situation.
All information we hold about you is confidential and will only be shared, with your permission, to other organisations that can help you.
