Carers Health Project
The Health Project works with health agencies in Leeds to improve support for Carers in the health care setting, such as GP Practices and hospitals. We also work in partnership with the Leeds Primary Care Trust with a view to improving communications.
We aim to ensure that GP Practices can identify Carers and, where appropriate, make referrals to relevant agencies including Carers Leeds. We recommend ways in which the GP surgery can support Carers within the practice. This involves providing Carer awareness training to staff at the practice amongst other things.
We produce a range of resources to improve communication in health centre, clinics and on hospital wards. Our green referral cards encourage Carers to refer themselves to Carers Leeds services and they are included in a Carer Discharge pack available on hospital wards.
