Carers Hospital Outreach Project (CHOP)
Provides support to Carers in Wandsworth looking after someone when they leave hospital.
If you are looking after somone when they leave hospital, CHOP may be able to help you by:
- offering a listening ear at any time during the hospital stay
- advising on your rights
- supporting planned admissions for yourself or the person you care for
- linking you into the Wandsworth Carers' Centre
- providing information on cummunity support services
- supporting you at discharge planning meetings
- provding information on welfare benefits
- offering an opportunity to discuss all your future caring options
WE CAN MEET YOU AT HOSPITAL OR VISIT YOU AT HOME
Before deciding whether you are willing and able to care you may want to find out:
- which staff have a responsibility for discharge planning
- the support services available following discharge
- information about the illness or disability
- details of any medication, its side effects and repeat perscriptions
- what to expect in the future
- any need for special aids or equipment at home
- plans for follow up and future appointments
- transport arrangements from hospital
