Health Liaison GP Link Project

This service has information desks regularly at 21 GP Practices within Harrow as well as the now familiar twice weekly desk located at the main entrance of Northwick Park Hospital.

This service enables carers to identify themselves and signposts them to services which will most appropriately meet their needs. The Health Liaison GP Link worker advises GP Practices of the needs of carers within their practices and is currently carrying out training courses for practice staff which is a programme being run by the Primary Care Trust.

The Health Liaison GP Link worker liaises regulary with the staff at Northwick Park Hospital. This liaison and information sharing is directly helping carers as this will often lead to referrals from Medical Staff. Another important part of this project is raising awareness of the needs of carers within the Health Service, training for practice staff, student nurses etc.

This project has now expanded to include a Pharmacists Co-ordinator. Tunde's job is to encourage pharmacists to help people understand that they are carers and steer them in our direction so that we may be able to support them in some way. The pharmacists has been extremeley helpful, committing to putting fliers and emergenecy cards into their prescription bags.
